8/7/2023 EnchantaVaughn Approved Behavior: Upon arrival, Ben-Nita was observed helping her mom with different chores around the house (i.e. washing dishes, folding clothes, taking out the trash, and cleaning up her room). Ben-Nita appeared genuinely happy and energetic when greeted by QMHP-C. Ben-Nita was cheerful and expressed good manners. Behavior: Ben-Nita transitioned into the next activity without conflict and proceeded to engage into meaningful conversation. Ben-Nita expressed that she wanted to look into more activities that could benefit her and help her deal with her stress at home better. Ben-Nita expressed positive thoughts about herself stating “I feel amazing”. Behavior: During the second portion of the session, Ben-Nita appeared to be a little irritated with her mother as well as QMHP-C when discussing ways to control her behavior to prevent her from getting in trouble in the home daily. Intervention: QMHP-C conducted family session. QMHP-C debriefed and inquired about her daily experiences. QMHP-C supported Ben-Nita making good decisions and having positive behaviors in the home. QMHP-C expressed gratitude toward positive interaction with mom and sibling. QMHP-C supported and monitored Ben-Nita in social and academic setting to ensure she was gaining insight on her mental illness. Intervention: QMHP-C supported Ben-Nita involvement in selected activities. QMHP-C processed with Ben-Nita about what she learned from activity. QMHP-C praised Ben-Nita for her cooperation. QMHP-C encouraged Ben-Nita’s mother to provide frequent positive reinforcement to maintain Ben-Nita's interest and motivation in maintaining positive behaviors and completing her “at home” school assignments and/or chores. QMHP-C praised Ben-Nita for listening and following directions and interacting safely during each intervention. QMHP-C observed Ben-Nita complying with rules and expectations. Intervention: QMHP-C discussed following rules and regulations and ask mother about how often she has to talk with Ben-Nita before she listens. QMHP-C talked with Ben-Nita about expressing her thoughts and feelings in a more constructive manner through more constructive actions. QMHP-C discussed sibling rivalry and alternative measures to confrontation and being argumentative with each other. QMHP-C discussed family dynamics and assessed environmental stressors. Response: Ben-Nita said she woke up feeling good and was ready to have a good day. Ben-Nita expressed positive thoughts about herself. Ben-Nita stated she didn't need help with anything at the moment but would say something if she needed help. Ben-Nita communicated by nodding and exhibiting good eye contact. Ben-Nita verbalized positive feelings. Ben-Nita displayed respectful, non-aggressive behaviors in the home. Ben-Nita was pleasant and cooperative and demonstrated a willingness to discuss the factors contributing to her desirable mood. Ben-Nita was able to process making good choices with QMHP-C and her mom. Response: Ben-Nita displayed good participation and was responsive to all instructions, interacting well with mom and QMHP-C. Ben-Nita responded in a positive manner acknowledging the positive reinforcement that the QMHP-C provided. Ben-Nita responded well to feedback and discussed some things she can do better when it comes to new challenges. Ben-Nita acknowledged interventions and verbalized clear understanding of each intervention. Response: Ms. Shabazz verbalized that she has to constantly talk with Ben-Nita and redirect her on several occasions daily because she knows that she misses her mother. Ms. Shabazz as well as Ben-Nita reported that when Ben-Nita gets upset when she do not have her way and sometimes walks away from her. Ben-Nita admitted that she gets upset immediate after being told "no".
